Over the past fifteen years, the growing recognition of systemic hypertension in pets has underscored why blood pressure should be measured in a systematic, standardized way. The correlation between invasive and noninvasive techniques has been extensively documented in both anesthetized and conscious patients.

Invasive monitoring remains the most accurate way to measure blood pressure. In day-to-day veterinary practice, however, it requires catheterization and sedation or anesthesia, and carries the risk of complications such as infection, vasospasm, and thrombus formation. Indirect, or noninvasive, blood pressure (NIBP) measurement, on the other hand, is regarded as reliable and consistent when recorded under standardized conditions.

Noninvasive methods

NIBP monitoring techniques are the preferred choice in routine clinical practice because they are more practical to use and can be repeated at short intervals, whether through Doppler or automated oscillometry.

Doppler vs. oscillometry, head to head

Doppler has been the more widely used method, in part because the oscillometric approach matured more gradually. Recent studies, however, show that Doppler readings diverge from invasive blood pressure in animals weighing less than 5 kg, which complicates therapeutic decisions and lowers diagnostic accuracy.

Automated oscillometry once drew conflicting conclusions about the accuracy of its readings, but as the equipment has improved and comparative studies against Doppler have accumulated, its use is now expanding.

The latest generation of oscillometric monitors offers greater sensitivity than older systems and brings several advantages over other noninvasive methods, Doppler among them.

How oscillometry works

The market already offers devices designed for veterinary practice with high-resolution oscillometry that let you assess reading quality in real time through the oscillometric waveform. With the oscillometric method, the monitor inflates the cuff automatically and, as it deflates the system, measures pulse oscillation and amplitude to determine systolic and mean pressure, then calculates diastolic pressure from those values.

What the evidence shows

Seliskar and colleagues (2012) caution that, in a study in dogs, the Doppler method did not meet the ACVIM criteria for reliable mean and diastolic blood pressure readings. Jepson and colleagues (2005), comparing Doppler and oscillometric readings, found no significant differences between the mean systolic pressure values; for diastolic pressure, however, the oscillometric method proved statistically superior.

Moll and colleagues (2018) highlight the lack of accuracy of the Doppler method in animals weighing under 5 kg, and warn that such readings should be interpreted with caution to avoid jeopardizing the patient's overall health, given the risk of overestimating blood pressure and misreading hypotensive patients.

Final considerations

It is worth remembering that reliable, trustworthy readings depend on an operator who is confident with the equipment. Knowing how to choose the measurement site, the right cuff size, and the patient's positioning, as well as accounting for the animal's emotional state, are all factors that must be considered.
