Veterinary Holter monitoring: what it is, how it works and when to use it

Veterinary Holter monitoring is an ambulatory electrocardiographic test that continuously records an animal's heart rhythm, typically over 24 to 48 hours, while the patient follows its normal routine at home. Because it captures the heart during activity, rest and sleep, it reveals intermittent arrhythmias that would rarely show up on a brief in-clinic electrocardiogram. Interpretation and the decision to order the test always rest with the veterinarian.
How the test works in practice
The device is a lightweight portable recorder connected to electrodes attached to the skin of the chest. The set is usually held in place by a vest or elastic wrap so it does not interfere with the animal's movement. Recording continues throughout the agreed period, capturing thousands of beats that would never fit on a conventional tracing.
While the device is worn, the owner notes activities and relevant events in a diary, such as walks, meals, sleep or episodes of weakness and fainting. This diary makes it possible to correlate symptoms with what the heart was doing at that exact moment, a decisive piece of information for interpretation.
At the end, the recording is processed by analysis software and reviewed by a qualified professional, in person or via telecardiology. Most dogs tolerate the vest well; in cats the technique is feasible mainly in medium to large animals with a calm temperament, assessed case by case.
Syncope and fainting episodes
Working up syncope, the transient loss of consciousness, is one of the classic indications. Because episodes tend to be brief and unpredictable, they rarely coincide with an in-clinic electrocardiogram, which makes prolonged recording especially useful.
In a retrospective analysis of 44 dogs with syncope, an arrhythmia was implicated as the cause of the episode in 30% of recordings and ruled out as the cause in 12%, illustrating how the test helps both confirm and exclude an arrhythmic origin. The clinical weight of each finding, however, always depends on its correlation with the symptoms noted in the diary.
Intermittent arrhythmias
Many arrhythmias come in bursts and vanish within minutes. A premature beat heard on auscultation, or an irregularity caught on a short tracing, may not tell how frequent or severe the phenomenon is across the day.
Holter monitoring quantifies events over 24 to 48 hours: the number of ventricular premature complexes, the presence of pairs, triplets or ventricular tachycardia, and how the rhythm behaves during exercise and sleep. That objective count is the foundation on which the veterinarian decides the management plan.
Assessing antiarrhythmic therapy
Another important use is measuring the response to an antiarrhythmic that has already been started. By comparing counts before and after treatment, it is possible to estimate whether the arrhythmia burden has dropped significantly.
An essential caution applies here: the frequency of premature beats varies widely from one day to the next. In Boxers with more than 500 ventricular premature complexes in 24 hours, spontaneous variability of around 80% between recordings has been described, with no medication at all. A small reduction may therefore reflect only natural fluctuation, and any reading of a drug's effect must always be prudent and individualized.
Subclinical cardiomyopathies
In predisposed breeds, Holter monitoring is central to screening for heart disease that is still silent. In Doberman dilated cardiomyopathy, the European Society of Veterinary Cardiology guidelines recommend starting screening at three years of age and repeating it annually, combining Holter and echocardiography, since a single test does not exclude future development of the disease.
In those guidelines, fewer than 50 single ventricular premature complexes in 24 hours is considered normal, although any number warrants attention; 50 to 300 on two recordings within one year, or more than 300 in 24 hours, are compatible with the occult phase of the disease. In Boxer arrhythmogenic cardiomyopathy, it is similarly described that more than 300 complexes per day and/or ventricular tachycardia suggest the disease, while the intermediate range usually calls for a repeat test. The exact values and their application depend on the breed and the veterinarian's clinical judgment.
24 or 48 hours, and the role of connected monitoring
Recording duration is a clinical decision. Twenty-four hours is often enough for an initial screen, while 48 hours, or longer recordings, increase the chance of capturing rare events and offset part of the day-to-day variability. The choice balances the probability of catching the event, the animal's tolerance and the purpose of the test.
Holter monitoring does not replace the resting electrocardiogram or the echocardiogram; it complements them, each answering a different question. When electrocardiographic monitoring is integrated into a connected platform, the recording flows from the device to the analysis software and to the cloud, with telecardiology review, shortening the path between capturing the signal and placing a qualified reading in the veterinarian's hands.
